Irritable bowel syndrome
Irritable bowel syndrome (IBS) is the most common intestinal upset, affecting 1-2 in every ten people. IBS is not dangerous in any way. It can be uncomfortable, unpleasant and distressing.
IBS symptoms vary from one person to another. Symptoms include vague abdominal discomfort, a sense of fullness sometimes called bloating, spasm-like pain in the tummy that comes in waves, constipation in some, and loose motions or diarrhoea in others.
Irritable bowel syndrome symptoms are intermittent. They come and go.
Symptoms may be present for few days at a time, or a week or longer. They usually settle down completely between flare-ups only to come back later. Irritable bowel syndrome is triggered in some people by stress. Some people can identify foods associated with a flare up of their symptoms.
IBS symptoms may be anything from occasional and mild through to severe distressing and incapacitating. The pain can be severe and very real.
Causes of IBS
There is no known cause for IBS. The condition is generally considered to be of unknown origin.
Caution with self-diagnosis and treatment
IBS has certain recognisable features that help to confirm the diagnosis:
- People usually have IBS on and off for years.
- IBS usually starts in the 20-30s.
- The pattern of symptoms of IBS are usually similar from one flare up to the next.
- People with IBS are usually perfectly well and symptoms free between flare-ups.
Symptoms to report to your GP (where self-treatment is not recommended)
- Weight loss (people with IBS usually do not lose weight).
- Constant pain or increasing pain in one spot (the pain of IBS usually comes and goes during a flare up and disappears completely between flare ups).
- Worsening constipation suggesting a blockage of the bowel.
- Blood in the motions (stools). IBS can cause diarrhoea but not blood in the stool.
- Symptoms starting in people over 50 (IBS usually starts early in life although it may be a life-long complaint).

Treatments: Colofac, Fybogel, Colpermin
We carry out consultations and prescribe medicine to reduce spasm in the bowel and sooth the bowel.
Colofac contains mebeverine, which is an antispasmodic.
Fybogel with colofac contains mebeverine plus fibre. For some people this works better than Colofac on its own.
Colpermin capsules contain the natural soothing ingredient, menthol.
POMColofac (mebeverine) and Fybogel with colofac are prescription only medicine.
Preparations
- Colofac, Mebeverine 135mg per tablet, taken three times daily before meals.
- Colofac MR, Mebeverine 200mg slow release tablet taken twice daily.
- Fybogel Mebeverine, Mebeverine 135mg plus ispaghula husk 3.5g one sachet twice daily
- Colpermin capsules. Peppermint oil 0.2mls per capsule (include peanut oil) 1-2 capsules three times daily.
Other things that help in some people but not others are: eating more fibre, eating less fibre, avoiding foods that seem to be associated with a flare-ups, trying to reduce stress, drinking plenty (minimum of 1.5L/day), and possibly for some people probiotics.
Side effects
Significant side effects are rare. They include allergic reactions after taking the first dose with mebeverine. Colpermin can cause heartburn and soreness around the anus. Colpermin contains menthol which can inflame the mouth if capsules are chewed.
